All students participating in interscholastic sports programs, regardless of grade level, must have a physical exam before participating in any sport. Physicals are good for one calendar year and must be valid on the first day of the season.

Physical examinations may be completed by the school physician (free of charge) and/or the student's personal physician. If a physical examination is completed by the student's personal physician a copy must be on file with the school nurse.

Dates, times and locations of the sports physical exams are published in the school calendar, Hill & Vale and sent home in quarterly newsletters. Along with an updated physical, a student must complete a health history questionnaire (pink form) prior to every season of participation.

Above you will find links to each of the forms for you to print and fill out. All paperwork pertaining to the student's physical examination and health history must be turned in to the school nurse for verification and approval prior to athletic participation:
